STEPS TO FOLLOW IF YOU HAVE A CLAIM

This is a summary only; please always carefully read the terms and conditions of your policy or certificate of insurance.

  1. Ouch:
    • If you believe you've had an Accident or Injury for which O.R.I. benefits are available, you must notify us within 20 days (or as soon as practical) by submitting a Notice of Loss.
    • Submitting a Notice of Loss is easy – just select the "Notice of Loss" option from the menu to the right of this page, fill in the required information, and click on "SUBMIT".
  2. Now keep an eye on your email:
    • Within 15 days of receipt of your Notice of Loss, we'll send you a confirmation of receipt.
    • In our confirmation, we'll attach the form you'll need to file Proof of Loss.
  3. Now for getting your claim processed:
    • Within 90 days of first receiving treatment for an Accident or Injury, you must provide us with sufficient Proof of Loss by submitting the required Proof of Loss form together with any requested additional information.
    • If you fail to send in a Proof of Loss within 90 days, please call the ORI Claims Services Department at (800) 574-7117 or send us an email at claims@GetORI.com. You may be eligible for an exception to the 90-day requirement under certain circumstances.
    • Send your Proof of Loss together with requested information to: Security National Life Insurance Company, O.R.I. Claims, P.O. Box 57007, Salt Lake City, UT 84157-0007.
  4. What if:
    • Even if for some reason you don't receive a confirmation of receipt of your Notice of Loss together with the Proof of Loss form, you can still simply access the Proof of Loss form by selecting the "PROOF OF LOSS" option from the menu to the right of this page.
